The information contained in this directory may change during your plan year. A provider at times may no longer participate in the network, or their tier status may change resulting in different cost sharing. Soon after the provider changes their status, out-of-network penalties or increased cost sharing will result. Please call the customer service phone number on your ID card to learn more about the participating providers in your network(s) and the implications, including financial, if you decide to receive your care from non-participating providers.
