Transition of care

If you are new to UMR and are currently receiving treatment for a specific medical condition by an out-of-network provider, we don’t want to disrupt that care.

That’s why we offer Transition of Care, which lets you continue to receive care - for a limited time - as we help you make the change to a network provider.

To learn more about Transition of Care, and to learn if you qualify, download this helpful form and application.
